Reopening a Worker’s Hand: From Frozen Grip to Restored Independence

Manual worker lost his ability to work when burn scars locked his hand into an immovable fist.

A manual worker’s hands are their identity, their trade, and their survival. For the man, a burn injury healed with dense palmar contractures that steadily pulled his fingers inward toward his palm. As weeks became months, the rigid scar tissue tightened into an unyielding biological lock, freezing his hand into a permanent partial fist.

He could no longer grasp tools, carry heavy materials, or manage basic daily chores without acute discomfort. The inability to work stripped him of his daily wages and pushed his dependent family into acute financial distress.

The Mechanical Trap of Palmar Contractures

Palmar skin is uniquely designed for grip and tactile sensation. When burns heal by secondary intention without specialized pressure splinting, myofibroblasts relentlessly contract the wound bed. In the palm, this forces the metacarpophalangeal and interphalangeal joints into fixed flexion deformities.

Without surgical intervention, prolonged flexion causes the collateral ligaments of the fingers to shorten permanently, causing joint stiffness that physical therapy alone can never reverse. Yet private reconstructive surgery remained completely unaffordable for his household.

The Surgical Intervention at Prakriya Hospital

BHF identified his case through our partner network in Karnataka. Under the clinical guidance of Dr. Rajeev B Ahuja, the man was immediately scheduled for full reconstructive sponsorship at Prakriya Hospital in Bengaluru.

The surgical team excised the thick palmar fibrous bands, releasing the tightened volar plates and restoring full passive extension to the trapped fingers. A full-thickness skin graft was placed across the defect to resurface the palm, followed by customized splinting and physiotherapy.

A Livelihood Restored

Following supervised rehabilitation, the man has regained full active grip and release. He has returned to active work, providing for his family with dignity, his hands once again capable and free.
